- Disability certificate (for PwBD category candidates) in the prescribed format issued by the Competent Authority.
- Income and Asset Certificate (for EWS category candidates) in the prescribed format issued by the Competent Authority.
- 2 recent passport size photographs.
- Candidates employed in Government/Quasi-Government/PSU must send applications through proper channel or produce NOC at the time of test/interview.
- Pre-Employment Medical Examination must be done in a Central/State Government Hospital (minimum District Hospital status).
